Use this form to let us know how the end of the semester looks for you and your students.

When is your last available day this semester?

How will your students be wrapping up the semester?
*
Progress Assessments are required for private lesson students unless they perform on one of the April or May Recitals or are exempted by a staff member. Common reasons for exemptions include: Late start in the semester, recent instrument change, learning difference accommodations...etc. The options for wrapping up the semester are: Progress Assessment, April Recital Performance, May Recital Performance or Exempt
They will be doing progress assessments
They will be playing a solo on the recital in April
I think they should be exempt from assessments
Each student will be doing something different (Ex: 1 = progress assessment and the other = recital)
They will be playing a solo on the recital in May
When would you like them to have their assessment (if applicable)?
